Lasik

The technique of the Lasik ( in situ Kératomileusis ) is an surgical operation of the eye. Its goal is mainly to correct the Myopie, but also of other aberrations like the Hypermétropie, the Astigmatisme or the Presbytie (one speaks then about Presbi-lasik by modifying the form of the Cornée).

Basic principles

The operation is practiced under local Anesthésie (topic by drops). The first stage of the intervention consists in cutting out a surface corneal shutter (90 has 180µ) retained by a hinge usually with 12:00. The laser can then work on an internal zone of the cornea (stroma) without deteriorating the membrane of BOWMAN and to modify it by photo-ablation. When the intervention with the Laser (which lasts about a minute) is finished, the shutter is positioned back by the surgeon.

The average current of cutting of the corneal shutter is the use of a Microkératome, apparatus mechanical that the surgeon applies to the eye.

An operation 100% laser is now possible. Cutting is then carried out thanks to a laser " femtoseconde" (FS) in a few tens of seconds, then ablation is carried out using a Laser excimer. Risks related to the character " mécanique" cutting are thus reduced here, and the characteristics of cutting (angle, thickness, homogeneity,…) are controlled better.

Side effects

Side effects (halations, Photosensitivity, etc) are usually noted. They appear in a more or less transitory way, in the days following the operation.

The " side effects; directs" , appearing during the hours following the operation, are: the feeling of irritation, impression to have a grain of sand in the eye, the perception of a " voile" … A few hours later, or the following day, the vision reaches 80% of the end result in general. However, during several weeks or several months, the patient can feel the following side effects, in a more or less transitory way: ocular dryness, loss of contrasts, acuity reduced in a dark environment accompanied by visible halations around the sources of light or by extremely contrasted zones, etc

More important side effects can intervene, like the unfolding of image, or a post-operative infection.

Good number of side effects, like their evolution in time, are conditioned by the operational tools. Thus, the most modern means to cut out the cap which is the laser femtoseconde, theoretically reduced probability and intensity of the post-operative embarrassments.

In the same way, of the personalized programs of treatment, although more expensive, find their interest in the reduction of these side effects or their duration, since ablation carried out is optimized according to the treated eye. One can for example, to choose a treatment optimized with respect to the aberration S, or a " treatment; tissue saving" , optimizing the intervention to minimize the thickness of ablation.

Lastly, these side effects, if they end up persisting, can, for some of them, being treated by one second operation, in the months following the first.
